Nordic charter for adolescents rights concerning transfere from pediatric to adult care

This is a new charter which NOBAB intends to implement and deploy within the next years. It takes into consideration the fragile and delicate period in a sick or disabled teenagers life when he or she needs ongoing care from the healthcare system. He or she has to leave the security of the petriatic care so familiar to them and move into the unknown healthcare world of the grown ups.

The aim is that within two years the content of the articles will be a normal part of the transfere

1. Continuity

Adolescent which because of their illness and/or functional disablity need ongoing care have right to continuity in healthcare service

2. Preparation

With good notice adolescents have the right to actively participate in the preparation for the transfere to adult care.

3. Information

Adolescents have the right to get information about consequences of the transfere to adult care

4. Cooperation

Adolescents have the right to actively participate in all descisionmaking concerning the tranfere of care to adult care

5. Respect and integrity

Adolescents integrity must be respected concerning the tranfere of care to adult care

6. Qualified personel

Healthcare workers who take over treatment and care of adolescents must have the required educations and qualifications to do so
